Improve the transition from mental health child service to adult

What the petition asks
As it stands the mental health services in the UK discharge young people at the age of 18 and in some cases even 17 and move them onto the adult services. This process is in most cases tricky and many get lost in the system. The system needs a change to help the people who need it.
