Reform. Statements signed as true by the person who gave the statement. Record interviews. Retain recordings as a minimum until the report is signed as accurate so it can be used to confirm discrepancies if any.
Statements given to police are signed as true. Child welfare report statements should also be signed. Both are used in court.
