Require clinicians to review full patient history before diagnosis or discharge

What the petition asks
Legislate to introduce a new requirement for clinicians to review a patient's full medical history, when presenting with chronic or repeating symptoms, before diagnosis, discharge or treatment decisions.
This should be known as Faye's law and could improve patient safety and continuity of care.
We believe many patients can be misdiagnosed or dismissed because their full medical history is not reviewed. This can lead to missed warning signs, repeated errors and avoidable harm. Faye’s Law would require clinicians to consider full history before diagnosis or discharge, which could improve patient safety, safeguarding and continuity of care. It could also help prevent mislabelling, reduce preventable deaths and restore trust for patients and families.
