Review NHS Guidance to Protect Vulnerable Adults in Community Care

What the petition asks
We are calling on the Government to review NHS guidance to ensure current practices prevent deaths caused by systemic failures in emergency triage and community healthcare coordination.
We would like the guidance to set out a “checklist” of expected care to ensure no other family suffers preventable tragedy:
1. Mandatory Clinical Triage Review
2. Mandatory Inter-Agency Welfare Checks
My mother’s death was not just a medical event; it was a failure of the safety net. We think this NHS wide guidance will turn "missed opportunities" to provide excellent care in community settings into "mandatory actions" which can save the lives of the most vulnerable members of our society.
