What the petition asks
Patients may experience fragmented or delayed care because NHS trusts use different record systems. We want a single mandatory NHS-wide digital patient record to help prevent errors or lost information, and stop patients being triaged or treated without full medical history across all NHS hospitals.
I am starting this petition based on lifelong personal experience navigating NHS care for myself and while advocating for my family. Over many years we have repeatedly seen vital medical details lost, delayed, not recorded, or not shared correctly between trusts. In our experience this has led to fragmented care, repeated assessments, longer waits, unsafe triage, and clinicians making decisions without full context. We believe a single mandatory NHS-wide digital record would help improve safety, efficiency, and patient outcomes.
