South Lanarkshire Health and Social Care Partnership Triumphs at the Scottish Health Awards

The motion

That the Parliament congratulates South Lanarkshire’s Health and Social Care Partnership (SLHSCP) on winning the Integrated Care category at the recent Scottish Health Awards for their Home First approach; notes that the Scottish Health Awards celebrate excellence across health and social care services; understands that SLHSCP’s Home First has been described as taking a "Goldilocks approach", as the key aim is to deliver care that is "just right", as opposed to too much or too little, and that the judges were impressed at both the progress already achieved and the longer-term plans for Home First; further understands that Home First is based on compassion and coordinating care around an individual’s needs, where assessments are conducted in familiar environments whenever possible, reducing stress and allowing for greater accuracy of care need; recognises that multi-agency Home First teams have been established in Lanarkshire's hospitals, with teams integrated into the discharge without delay programme and the End of Life pathway team, which provide holistic care to patients approaching end of life within a homely setting, and believes that the targeted care approach of Home First is vital given the pressures and strain it considers are being experienced across the health and social care system.
