British Red Cross, Life Beyond the Ward Report

The motion

That the Parliament welcomes the report, Life beyond the ward – recommendations for improving hospital discharge in Scotland, published by the British Red Cross, which explores the challenges people can experience on their discharge journey from hospital to home; understands there is considerable variation in discharge practices across hospitals; recognises what it considers a lack of co-ordination and communication between hospitals, primary care and social care; appreciates that challenges remain in accessing social care packages and wider support; considers that a potentially significant proportion of people who are deemed clinically fit to leave hospital are sent to a home inappropriate for their recovery; supports the report recommendations to fulfil the transition to a more preventative approach to care by improving the funding of community care and shifting focus towards people and their holistic needs; believes that harnessing the power of non-clinical support, including the voluntary and community sector, can support effective discharge; considers that checks on people’s non-clinical independence factors as part of the discharging process of patients through a five-step independence check should be introduced, and calls for the Scottish Government to adopt these recommendations to improve discharge and support some of the most vulnerable people in society to maintain independent living.
